We hereby apply for credit with Pest Pro and certify that the information provided is current and correct. We agree to notify Pest Pro at the address below if any information on this application changes and authorize Pest Pro to verify any information provided. We understand that the terms of credit are NET 30 (thirty) days from the date of invoicing, and we agree to meet these terms. We understand that payments received by Pest Pro after thirty days will be considered late. We recognize that at 31 days after invoicing, invoices are subject to a minimum $25 late fee plus a 2.5% monthly financing charge on the total amount of all past due balances, in addition to any fees, charges, or other expenses incurred in a collection process. We recognize that projects exceeding $5,000 may require a prepayment covering direct costs required to complete the job.
